1. Residents must sign a separate Residency Agreement outlining their rights, obligations, and fees.
2. Services provided, including care plans, are subject to availability and the agreed terms.

2. Late payments may incur penalties or result in service disruptions.

1. Residents, families, and visitors must comply with facility rules and regulations, including health and safety protocols.
2. Encino Terrace reserves the right to deny access to anyone violating these rules.

1. Residents are required to provide accurate medical information.
2. We may require health assessments for safety and care planning.

1. Encino Terrace is not liable for personal property loss, injury, or damages except as required by law.
2. Services are provided “as-is” and may be subject to availability or unforeseen circumstances.

1. Personal and health information is managed per our Privacy Policy.
2. Residents and families must respect the privacy of other residents and staff.

1. Residency agreements can be terminated per the terms specified in the agreement.
2. Grounds for termination include non-payment, non-compliance with rules, or endangerment of residents or staff.
